1. To replace your Voter Information card, click here.

- To access the Voter Information Card replacement form in Spanish, click here and select “Español”, then “Register or Update”.
2. Under “Reason for Application” select “Request to Replace Voter Information Card”.
3. Answer the eligibility questions and complete the application form to submit your request for a replacement Voter Information card.
